A home in the community.
Support for the life ahead.

Our assisted living communities provide personal care, clinical and behavioral support for adults with complex needs. Our mission is to help people thrive in the community and receive support through periods of crisis.

We work alongside residents, families, case managers and treating providers to support transitions into community living and build everyday skills. Individual plans reflect each person’s routines, choices, relationships and goals.

Medical and behavioral health treatment is coordinated with each resident’s treating providers.

A team beside you.

The right support starts with your routines, interests and goals. These Washington programs offer different kinds of help with life in the community. We’ll explain what is available at the community you’re considering.

Community Stability Supports

A personal plan for each day.

Help with daily care and medication, activities shaped around your interests, and a team that regularly reviews what supports you.

  • Your own activity calendar

A monthly calendar reflects your interests and goals. You or your representative help shape the plan, so activities have a place in your day that makes sense for you.

  • Choices across the community

The facility activity calendar offers choices throughout the week, with group and individual activities at home and in the wider community. There is room for planned activities and spontaneous moments.

  • A team that stays involved

Care, nursing, behavior-support and activity staff coordinate around your plan. Monthly reviews consider what’s working and what needs to change. Your prescriber also reviews your medication plan.

CSS eligibility & referral details

CSS is a Residential Support Waiver service. Washington’s Department of Social and Health Services (DSHS) reviews age or disability, financial eligibility, current or anticipated need for a nursing facility level of care, medical and psychiatric stability, and specialized support and placement needs. People may be transitioning from a hospital or already living in the community.

This support is coordinated with other services and your crisis plan. Daily personal care and activities remain part of your broader residential plan.

CBHS eligibility & referral details

For adults 18 or older with qualifying Apple Health coverage and a qualifying mental health diagnosis. Financial eligibility, eligibility for home and community-based services, assessed needs and risk criteria also apply. DSHS Home and Community Services begins the assessment and referral. ECS

Expanded Community Services

A shared plan, consistent support.

Help with daily care and medication, with a behavior-support provider who works alongside the residential team.

  • Support that fits your routines

Your plan identifies what helps you feel settled and how staff can support you day to day. Activity options can also help you stay involved in life at the community.

  • A team working together

Facility staff and the behavior-support provider review your needs together, involve other care professionals as appropriate, and meet when your needs call for it.

  • Help through difficult moments

Your individual behavior and crisis plans guide how staff respond. The team connects with your case manager and providers when your needs change.

ECS eligibility & referral details

In assisted living, ECS is a Residential Support Waiver service. DSHS applies the waiver’s age or disability, financial, care-level, stability and complex-support criteria. The appropriate service follows the person’s assessed needs and authorized care plan.

Getting started

Start with a conversation.

You don’t need to know which program applies before contacting us. Start with the community you’re considering.

  1. Talk with an administrator.

Tell us about the person’s daily life, support needs and goals. We can explain the setting, available services and referral review.

  2. Arrange a care assessment.

A state care assessment helps identify the support a person needs. The responsible state agency decides program eligibility. Services are then authorized by the state or health plan, as applicable.

    Request a DSHS assessment
  3. Plan ongoing support.

Bring daily care, activities, behavior and crisis plans, medications and connections with treating providers into a coordinated plan for community living.

Program eligibility, service authorization and admission are separate decisions. Available support depends on assessment, authorization and the selected community’s ability to meet the person’s needs. The linked agency criteria provide the full requirements.

2018

Tekoa Care Center participated in a Washington DSHS pilot that included Community Stability Supports. That experience informs our commitment to community living today.
